I would say it's most likely a wiring issue. You'd want to get under the trailer and make sure the brake output circuit wire is ran to all of the brake assemblies and then also grounded properly. Most likely the wiring has made it to the middle axle but some type of short or cut wire is preventing it from getting to the front and rear axles. When we see the problem you described with an electric over hydraulic actuator it is almost always because the brake controller being used is an integrated brake controller that is not designed to work with EOH actuators. If this problem persists the actuator can end up getting damaged.

I took a look in the online version of the owner's manual for your 2011 Ford F-450 Super Duty and it looks like the right turn/stop is fuse location 13 while the left turn/stop is fuse location 14 in your passenger compartment fuse box.
In the power distribution box (under the hood) the left turn/stop is location 74 while the right turn/stop is location 75. I recommend verifying this in your copy of the owner's manual.
